- 2 reference letters
- New York State or Government Photo ID
- Social Security Card
- A recent physical exam report including the followings
- A recent negative PPD/Mantoux results or a negative Chest X-ray
- A recent drug screening
- Rubella and Rubeola titres showing immunity
- Work Permit or green card
